Notice regarding disabled parking permits

Our office is happy to assist patients with disabled parking permit requests when medically appropriate. In most cases, temporary permits are issued unless the physician determines that a permanent permit is medically necessary.

Please note: Patients requiring parking permits due to recent knee replacements, orthopedic injuries, or other orthopedic conditions should request these forms from their orthopedic surgeon or treating orthopedic physician, as they are best able to determine the appropriate duration and level of disability related to those conditions.
